RN Field Case Manager – Home Health

Rockland, NY

We are seeking a dedicated RN Field Case Manager to join our growing home health team in Rockland, NY. In this role, you will provide high-quality patient care, coordinate with physicians, and supervise Home Health Aides (HHAs) while ensuring patients receive the best possible support in their homes.

• Perform home health admissions and reassessments per physician orders.
• Coordinate and supervise HHAs in patient care delivery and discharge.
• Develop and implement individualized care plans.
• Observe, assess, and document patient progress.
• Communicate with physicians and other healthcare providers regarding patient condition changes.
• Educate patients and caregivers on disease processes, medications, and treatment plans.
• Specialize in infusion care with provided training and ongoing skill development.

• Active RN license in NY (Required).
• Home health experience preferred , but we are willing to train nurses without OASIS experience.
• Pediatric experience is highly preferred but not required.
• Strong assessment and communication skills.

• Full-Time: Nurses are required to complete 33 points per week (Start of Care = 2 points, Subsequent Visit = 1 point).
• PRN Rates: $85 per revisit, $112 per Start of Care.
• Mileage Reimbursement.
